Gift Card Terms

CoreAge Rx gift cards are issued and redeemed by DSO Fine Shrimp LLC dba CoreAge Rx, 1309 Coffeen Avenue, Suite 1200, Sheridan, WY 82801.

The gift card does not expire. The underlying value does not decline over time and is not reduced by dormancy, inactivity, or service fees.

A gift card awarded as a giveaway prize is issued in the winner’s name, for the winner’s own use. It is non-transferable. It may not be sold, bartered, auctioned, or assigned to another person. A gift card offered for resale is void.

The gift card may be applied toward eligible products and services available for purchase through CoreAge Rx, including the product price, medical consultation and intake fees, membership or program fees, shipping, and applicable taxes, unless otherwise stated at checkout.

Certain products may be excluded where a manufacturer, pharmacy, payor, or applicable law restricts the use of promotional value. Eligibility is determined at the time of redemption.

A gift card is a form of payment. It is not a prescription, and it does not create any entitlement to a prescription product.

Prescription products require completion of the applicable intake process and evaluation by a licensed healthcare provider. Eligibility and treatment decisions are made solely by the provider in their independent medical judgment. Holding a gift card does not guarantee that any medication, including any GLP-1 medication, will be prescribed or will be available, and the gift card cannot override a provider’s medical decision.

If a provider determines that a prescription product is not appropriate, the gift card balance remains available for other eligible products and services.

The gift card may be applied to subscription and recurring-billing programs. The balance is drawn down as each billing cycle is charged.

When the balance is exhausted, an active subscription does not automatically stop. Any remaining amount due will be charged to the payment method on file and billing continues until the subscription is cancelled.

The gift card may be combined with generally available promotions and discounts unless a specific promotion states otherwise. It may not be combined with any offer that expressly excludes gift-card redemption.

The gift card is not refundable and unused value is not returnable for cash, except where applicable state law requires cash redemption — for example, where a remaining balance falls below a statutory threshold. Where such a law applies, a qualifying request will be honored as required by that law.

Partial balances remain available for later purchases. The card carries no implied warranty and has no value until activated.

The gift card is redeemable only for purchases through CoreAge Rx that ship to, and are available in, the fifty (50) United States and the District of Columbia. Product and prescription availability varies by state, and some products may not be available in your state.

Report a lost or stolen gift card promptly to support@coreagerx.com. A registered card with a verifiable remaining balance may be replaced at the issuer’s discretion. Value already redeemed cannot be restored, and an unregistered card cannot be replaced.

The gift card may not be resold, exchanged for cash except as required by law, used to purchase another gift card, used fraudulently, or used in violation of these terms or of applicable law. It may not be used after it has been reported lost, stolen, or obtained by fraud.

The issuer may void a gift card obtained through fraud, error, or in violation of these terms, and may decline redemption where it reasonably suspects misuse.

The issuer may update these terms where permitted by law. Material changes will not reduce the value of a gift card already issued.
