The prize is supplied by DSO Fine Shrimp LLC dba CoreAge Rx, 1309 Coffeen Avenue, Suite 1200, Sheridan, WY 82801(the “Prize Provider”), which issues and redeems the gift card. The Prize Provider does not administer this giveaway, does not select the winner, and is not responsible for its conduct.

This giveaway is promoted on FoodNoiseStop.com. It is not administered by, sponsored by, or associated with any social media platform, and any such platform bears no responsibility for it.

This giveaway is separate from any other promotion offered by Sponsor, including the $1,788 CoreAge Rx gift card giveaway promoted on medconsumerwatch.com. It has its own entry period, its own drawing, and its own prize. Entering one does not enter you in the other, and an entry in one does not affect the odds in the other.

No purchase or payment of any kind is necessary to enter or win. A purchase will not improve your chances of winning. Making a purchase from CoreAge Rx neither increases nor decreases the odds of being selected.

The giveaway begins on August 3, 2026 at 12:00:00 PM Eastern Time and ends on September 2, 2026 at 11:59:59 PM Eastern Time (the “Entry Period”). All times are Eastern Time. Entries submitted before or after the Entry Period will not be eligible. Sponsor’s clock is the official timekeeper for this giveaway.

Open only to legal residents of the fifty (50) United States and the District of Columbia who are at least 18 years of age at the time of entry. Employees, officers, and directors of Sponsor and its parent, affiliates, subsidiaries, and advertising or promotional agencies, and the immediate family members and household members of each, are not eligible.

Void where prohibited or restricted by law. Entrants are responsible for complying with the laws of their own jurisdiction.

During the Entry Period, submit your email address through any giveaway entry form on FoodNoiseStop.com and agree to these Official Rules. Entry is free. Providing a first name is optional and is used only to personalize correspondence.

Consent to receive marketing email is entirely optional, is collected separately, and is not a condition of entry. Declining marketing email, or later unsubscribing, does not cancel or invalidate a valid entry.

One (1) entry per person and per email address for the entire Entry Period. Duplicate entries will be disregarded and will not increase your chances of winning.

Entries generated by script, macro, bot, or any automated means, and entries using fraudulent, disposable, or misappropriated email addresses, are void and will be disqualified. Sponsor may disqualify any entrant who tampers with the entry process.

One (1) prize will be awarded: a $1,788 CoreAge Rx gift card. Approximate Retail Value (“ARV”): $1,788 USD.

The gift card is awarded to the winner. It is issued in the winner’s name, is for the winner’s own use, and is non-transferable.

The gift card may be applied toward eligible products or services available for purchase through CoreAge Rx, subject to the Gift Card Terms and applicable product restrictions.

No substitution, transfer, or cash equivalent is permitted, except that Sponsor reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable. Any portion of the prize not used is forfeited unless otherwise required by law.

Winning does not guarantee eligibility for, approval of, or access to any prescription medication.

Prescription products require completion of the applicable intake process and evaluation by a licensed healthcare provider. Eligibility and treatment decisions are made solely by the licensed healthcare provider based on their independent medical judgment.

The gift card cannot override a provider’s medical decision. Sponsor makes no representation that any particular medication, including any GLP-1 medication, will be prescribed, will be available, or will produce any particular result. No health, weight-loss, or treatment outcome is promised or guaranteed.

Nothing in this giveaway constitutes medical advice or the practice of medicine.

One (1) potential winner will be selected in a random drawing from among all eligible entries received during the Entry Period. The drawing will be conducted by Sponsor or its designated representative on or about September 4, 2026.

Odds of winning depend on the total number of eligible entries received during the Entry Period.

The potential winner will be notified by email at the address submitted with the entry, within 7 days of the drawing. The potential winner must respond within 7 days of the date the notification is sent.

If a potential winner does not respond within that period, cannot be contacted, declines the prize, is found ineligible, or fails to complete any required documentation, the prize may be forfeited and an alternate winner may be selected at random from among the remaining eligible entries. Sponsor is not required to award an unclaimed prize.

All entries and potential winners are subject to verification by Sponsor, whose decisions are final and binding in all matters relating to this giveaway. Sponsor may require a potential winner to provide proof of identity, age, and residency, and to execute an affidavit of eligibility and a liability and publicity release where lawful, before the prize is awarded.

All federal, state, and local taxes on the prize, and all other costs and expenses not expressly stated as included in the prize, are the sole responsibility of the winner. Where required, Sponsor may issue an IRS Form 1099 for the ARV of the prize and may request a completed Form W-9 before the prize is awarded.

Except where prohibited by law, acceptance of the prize constitutes permission for Sponsor to use the winner’s first name, last initial, and state of residence for promotional purposes without further compensation.

By entering, entrants release and hold harmless Sponsor and its parent, affiliates, subsidiaries, and their respective officers, directors, employees, and agents from any liability for any injury, loss, or damage arising from participation in this giveaway or from acceptance, possession, or use of the prize.

Sponsor is not responsible for lost, late, incomplete, misdirected or undeliverable entries or email; technical or network failures; or any error in the administration of the giveaway. Sponsor is not responsible for typographical errors in these Official Rules or in any giveaway materials.

Information submitted with an entry is collected by Sponsor and handled in accordance with the Privacy Policy. Entry information is used to administer the giveaway and, only where separate consent was given, to send marketing email. Marketing consent may be withdrawn at any time using the unsubscribe link in any marketing email.

These Official Rules are governed by the laws of the State of New York, without regard to its conflict-of-law provisions.

Redemption and use of the gift card are separately governed by the Prize Provider’s own terms — the laws of the State of Georgia, with disputes resolved by binding arbitration in San Diego, California under the then-current rules of Judicial Arbitration and Mediation Services, Inc. (JAMS).

Entrants waive the right to have disputes resolved as a class action, to the extent permitted by law, and waive all rights to punitive, incidental, and consequential damages.

If for any reason this giveaway cannot be conducted as planned — including infection by computer virus, bug, tampering, unauthorized intervention, fraud, technical failure, or any other cause beyond Sponsor’s reasonable control that corrupts or affects its administration, security, fairness, integrity, or proper conduct — Sponsor reserves the right in its sole discretion to modify, suspend, or cancel the giveaway, or to award the prize from among eligible entries received up to the time of the impairment.
